Actividades del grupo

Uveítis. Es la línea de investigación principal del grupo. En este campo estamos interesados en la patogenia del edema macular. En concreto estamos valorando el papel de la somatostatina como factor anti permeabilidad capilar y por tanto como potencial arma terapéutica frente a esta complicación.

También estamos interesados en el papel de la genética en la patogenia de las uveítis intermedia y posteriores inmunes. En este sentido vamos a iniciar un estudio multicéntrico junto con el Hospital Clínic de Barcelona, La Fe de Valencia, el hospital Clínico San Carlos de Madrid, el Hospital de Granada, el Hospital de Valdecilla y el CESIC en el que vamos a estudiar posibles genes candidatos implicados en estas enfermedades.

 

Unos de nuestros objetivos es obtener un producto celular capaz de regenerar la superficie ocular en casos de insuficiencia limbar total. El daño severo del limbo esclero-corneal es una patología ocular que afecta a las células madre del limbo o a su nicho, pudiendo provocar ceguera. Esta enfermedad es muy incapacitante y afecta a una población muy diversa, puesto que la causa puede ser tanto de origen inmunológico como ser consecuencia de traumatismos o quemaduras químicas en sujetos sanos. Además, implica importantes costes sanitarios a largo plazo.

La insuficiencia limbar precisa de una reparación de la población de células madre del limbo para obtener una recuperación duradera de la superficie ocular. Para ello, es necesario realizar un trasplante de las células madre al ojo afectado, ya sean provenientes de un individuo donante (cadáver o pariente vivo) (trasplante alogénico) ya sean del propio individuo (del ojo contralateral) (trasplante autólogo).

El cultivo de células madre del propio ojo del paciente permitiría la repoblación celular conservando la capacidad de auto-replicación celular. Asimismo, se evitaría el uso de medicaciones con alto grado de morbilidad necesarias para los trasplantes alogénicos, que en la mayoría de los casos fracasan, así como el peligro severo de descompensación del ojo inicialmente sano en casos de trasplante autólogo a partir del ojo contralateral.

Por otra parte, estudiamos el efecto regenerador y cicatrizante de distintos hemoderivados ricos en factores de crecimiento plaquetarios sobre la superficie corneal en casos de defecto epitelial.

La salud de la superficie ocular depende de la integridad del epitelio corneal. En casos de defecto epitelial corneal persistente, que no responden a los tratamientos estándar, se han descrito diferentes métodos para mejorar el tropismo de la superficie ocular. Entre ellos, y desde hace una década aproximadamente, se ha venido estudiando el efecto de preparados autólogos derivados de la sangre con un alto contenido en plaquetas, o en factores de crecimiento liberados por las plaquetas. Sin embargo, no existe consenso en el método de preparación de los mismos.

Los procesos de cicatrización son muy complejos y requieren la activación de múltiples vías de señalización implicadas en mecanismos tales como la homeostasis, inflamación, proliferación celular y remodelación tisular. Debido a la necesaria participación e interacción de distintos factores de crecimiento, estos hemoderivados ricos en factores plaquetarios ofrecen importantes expectativas para la regeneración corneal.

 

En los trastornos de aprendizaje, en concreto, en los trastornos de la lectura, siempre ha existido mucha controversia sobre su etiología, lo cual ha llevado a que exista un gran mercado de pacientes "desorientados". En ese aspecto, se ha dado un paso de gigante con las nuevas técnicas de imagen. Con nuestro estudio pretendemos demostrar que la dislexia tiene un origen neurológico y que en absoluto es un problema perceptivo. Para ello, realizamos resonancia magnética funcional en niños con dislexia y en niños con trastornos en la motilidad ocular, además de en un grupo control, para conocer los grupos neuronales que activan durante la lectura los diferentes grupos de niños. El objetivo final de nuestra investigación es saber si podemos utilizar la RMf como método diagnóstico de la dislexia.

Existe un grupo importante de niños con alergias oculares subclínicas, que son mal diagnosticados como tics y tratados como tal. Para corregir esa situación pretendemos desarrollar un kit de diagnóstico de conjuntivitis alérgica a través de la lagrima de los pacientes, para ello primero debemos de conocer los mediadores moleculares en lagrima de  la alergia ocular infantil. Por ello, realizamos un análisis de los posibles marcadores de alergia en niños con una conjuntivitis alérgica conocida y en niños control. Con posterioridad, una vez conocidos éstos, desarrollaremos un kit que sea capaz de detectarlos.

Tenemos una tercera actividad que consiste en verificar si somos capaces de forma prenatal de diagnosticar patologías oculares severas e intentar integrarlas en un marcador diagnóstico global de malformaciones. Para ello, se toman muestras de líquido amniótico en madres de niños que pueden tener patologías oculares hereditarias y valorar si detectamos la anomalía genética ya conocida en dicho líquido.
